Rates and Package Options

Pay Per Visit Rates:

Initial Evaluation = $150.00

Follow Up Visits = $125.00


Package Rates:

3 Visits: Initial Evaluation ($150) Plus 2 Follow Ups ($120/visit) = $390.00

5 Visits: Initial Evaluation ($150) Plus 4 Follow Ups ($115/visit) = $610.00

7 Visits: Initial Evaluation ($150) Plus 6 Follow Ups ($110/visit) = $810.00

9 Visits: Initial Evaluation ($150) Plus 8 Follow Ups ($100/visit) = $950.00


*All visits are up to 60-minutes long and are one-on-one with Dr. Richard Winters.  Dr. Winters only schedules one patient at a time. 

**A prescription for Physical Therapy from your doctor is required if your 'Plan of Care' should go beyond 30 days after the Initial Evaluation. 

**Upon request, an itemized list of services provided, also known as a SUPERBILL, can be emailed to you after each visit.  It is your responsibility to submit this document to your insurance company.  Reimbursement is not guaranteed, thus it is recommended that you contact your insurance company beforehand to determine whether out-of-network services are covered.
